Purpose:

The Collaborative Practice Specialist provides leadership, coaching/mentoring, role modelling and support for health care providers in developing excellence in clinical practice.   The Collaborative Practice Specialist demonstrates use of clinical knowledge and skills, critical thinking skills and excellent communication and interpersonal skills to collaborate with members of clinical practice committees, Clinical Educators, clinical and medical leaders and other appropriate disciplines and health care providers to develop, implement and evaluate the strategic direction for clinical practice, education, resource planning and delivery of patient care.

Duties:

  1. Maintains current knowledge of best practices for the delivery of care and/or service(s), including appropriate research, literature, accreditation requirements, policy and legislative requirements, and implementing improvements/changes as appropriate.
  2. Understands the current clinical practice expectations across the organization and identifies opportunities to initiate changes in accordance with pending practice changes.  
  3. Provide leadership in the integration and sustainability of evidence-based practice tools and processes to facilitate quality outcomes.
  4. Promotes an environment within the profession which supports clinical excellence, research, innovation and continuous learning and improvement.
  5. Promote understanding and application of evidence-based best practice clinical standards and practice expectations through a range of strategies.
  6. Identifies the need for new policies and practices, participates in their development or revision and provides leadership in implementation and evaluation of guidelines, professional standards and policies that impact patient care.
  7. Acts as resource to clinical staff, clinical educators and professional practice leaders to interpret and apply professional practice standards.
  8. Advocates for professional standards of practice using organizational processes.
  9. Liaises with CNO, Clinical leaders, Clinical Educators, Professional Practice Leads and Quality and Risk Management to ensure priorities for professional practice and education are addressed.
  10. Participates in appropriate staff development and professional education that support staff in maintaining continuing competence.
  11. Promotes clinical decision making with individual staff members and between departments.
  12. Supports the clinical and professional development of colleagues through mentoring and role modeling.
  13. Advances competencies, directives and other authorizing mechanisms to optimize scope of clinical practice.
  14. Incorporates standards for inter-professional collaboration in committee structures and processes.
  15. Provides consultation on corporate initiatives and processes as they may impact professional clinical practice and patient care.
  16. Maintains commitment to self-growth and professional development by remaining current in knowledge and application of best practice through the utilization of appropriate research and literature
  17. Demonstrates good interpersonal and conflict resolution skills.
  18. Demonstrates a comprehensive understanding of and ability to critically analyze roles, dimensions, and contexts of professional practice.
  19. Utilizes a variety of teaching/learning/communication strategies to meet the education needs of interdisciplinary staff.
  20. Work with clinical leaders, clinical educators, and professional practice leads to develop and review corporate and department clinical education.
  21. Participate on committees as assigned.
  22. Other duties as assigned.

Qualifications:             

  • Bachelor’s degree in a relevant clinical field and/or related Professional Certification
  • Regulated Health Professional, currently registered with a professional college, and in good standing

Experience:

  • Minimum of 5 years of recent diverse clinical experience, preferably with increasing levels of responsibility
  • Experience with creating and delivering training material

  • Knowledgeable in the Regulated Health Professionals Act, professional scope and standards of practice
    • Demonstrated record of strong, positive leadership in a clinical profession
    • Demonstrated ability to influence organization and professional directions
    • Demonstrated knowledge and skill in program development, evaluation and education
  • Demonstrated knowledge and skill in process management and continuous quality improvement
  • Demonstrated commitment to continuing education
  • Demonstrated proficiency in establishing effective interpersonal relationships, consensus building, conflict resolution, consultation and coaching / mentoring, both inside and outside the organization
  • Demonstrated proficiency in communication (written and verbal), group facilitation, problem-solving, complex analysis, change management and innovation
  • Demonstrated ability to foster and participate in research activities
  • Computer literacy in a Microsoft environment including use of technology to plan and document care and as a tool to support education
  • Human relations skills including empathy, sensitivity, understanding of human and organizational behavior, motivational techniques and counseling skills (e.g. teaching patients).

Ability:

  • Ability to set priorities, manage competing demands, demonstrate sound judgment and strong problem resolution skills.
