Purpose:
Sault Area Hospital is currently recruiting for a Registered Practice Nurse in our Program for Assertive Community Treatment (PACT). The successful candidate will be an integral member of the multidisciplinary mental health team and will liaise with the diverse mental health programs within the hospital and community following standards set by the College of Nurses of Ontario.
Duties:
- Effectively uses the Standards of Nursing Practice and the Nursing Process in assessing, planning, implementing and evaluating the care of the patient within the context of a multi-disciplinary team within the inpatient, outpatient and community settings.
- Collaborate and contribute to a client-centered team developed treatment plan.
- Implement strategies to address safety issues and mitigate risk for injury and/or death.
- Experience providing consultation to other multi-disciplinary team members from a psychiatric and general nursing perspective.
- Participate in discharge planning to ensure safe, effective and efficient discharges.
- Provide education and consultation to client and family, caregiver and other health care professionals in the hospital and community.
- Demonstrated ability to supervise, develop, revise and maintain pharmacological and medical treatment in collaboration with the team psychiatrist.
- Provide educational support to community partners and other hospital staff on psychiatric related topics.
- Document assessments, analysis, client goals, ongoing progress and according to the College of Nurses of Ontario Standards.
- Report pertinent information to other members of the health care team and participate in daily and weekly team meetings.
- Other duties as assigned.
Qualifications:
- Valid driver’s license and access to a dependable vehicle
- Current Certificate of Registration from the College of Nurses of Ontario, in good standing.
- Current knowledge and skills in Psychiatric / Mental Health Nursing.
Experience:
- Minimum of 2 years of psychiatric nursing experience required.
- Minimum 1 year of Community nursing experience.
Knowledge, Skills & Ability:
- Knowledge/ability to liaise with community resources
- Act independently in prioritizing patient needs to ensure clients receive appropriate intervention in a timely manner.
- Knowledge of risk assessments for mental health patients during office and community visits
- Demonstrated competence working independently and flexibly in a community-based setting.
- Ability to collaborate and contribute to a client-centered team-developed treatment plan.
- Demonstrated ability to maintain successful working relationships; ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner, both in person and on the telephone; tact, courtesy and alertness in public contacts.
- Substance abuse training and/or experience an asset.
- Prior experience with ACT program an asset.
- Knowledgeable in the Mental Health Act
- Knowledge and understanding of the hospital’s mission statement and core values.
- Must be able to accept direction and work with minimum supervision.
