Purpose:
Sault Area Hospital (SAH) is working in partnership with Canadian Mental Health Association (CMHA) to operate the new HART Hub in Sault Ste, Marie. This is a community driven response to the escalating challenges of homelessness, addiction, and mental health in our region. Rooted in compassion and collaboration, the Hub will serve as a centralized, accessible and inclusive space that supports the entire community from the most marginalized individuals to frontline service providers, community organizations and residents impacted by this crisis. This initiative is grounded in the understanding that these complex social issues affect everyone, directly or indirectly. By creating a hub that integrates health, housing, and social services, we aim to reduce barriers to care, improve outcomes and foster a more resilient and connected community to support those in need.
SAH will expand its current outpatient medication clinic into the HART Hub, providing education, support, symptom monitoring, and medication injections with the aim of improving medication adherence. This work will meet clients where they are at in the community at various locations based on needs.
Duties:
- In collaboration with the current outpatient medication clinic, provide direct care to individuals referred to the program including education, support, symptom monitoring and medication review.
- Conduct a nursing assessment and mental status exam of the psychiatric patient.
- Provide educational support to staff, patients, and family to provide support and education/health teaching regarding symptom management and understanding the medications
- Collaborative with the HART Hub staff, and other community providers, in the provision of care to individuals accessing services in the medication clinic
- Providing services to individuals at various locations in the community that include the HART hub, CMHA location Supportive Housing locations across the community
- Effectively use the Standards of Nursing Practice and the Nursing Process in assessing, planning, implementing, and evaluating the care of the patient.
- Use evidence based practice in the provision of education and patient care.
- Work closely with the psychiatrists/delegates, emergency physicians, emergency nurses, Crisis Intervention, family physicians, other interdisciplinary team members and community partners to provide care to the psychiatric patient.
- Comply with legislation, organizational and departmental, regulations, standards and procedures related to occupational health and safety
- Other duties as assigned
Qualifications:
- Current registration with the College of Nurses of Ontario in good standing
- Applied Suicide Intervention Training
- Non-violent Crisis Intervention
- Valid driver’s license and access to a dependable vehicle
Experience:
- Minimum of 3 years’ experience working in Mental Health and Addictions required
Skills:
- Excellent communication and organization skills
- Human relations skills including empathy, sensitivity, understanding of human and organizational behavior, motivational techniques and counseling skills
- Critical thinking and problem solving skills
Ability:
- Training and demonstrated ability to conduct a thorough nursing assessment of patient history, presenting illness, mental status and other relevant psychiatric assessments
- Ability to work independently and as a team member in a fast-paced environment in stressful situations
- Understanding and ability to implement requirements outlined in the Mental Health Act
- Demonstrated ability to maintain successful working relationships with an interdisciplinary team to achieve positive patient outcomes
- Ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner, both in person and on the telephone; tact courtesy and alertness in public contacts
- Knowledge of and ability to liaise with community resources
- Demonstrated Behavior consistent with Hospital Mission, Vision, Values and Standards of Performance.
