Position Overview

The Social Worker for Clinical Programs at Group Health Centre plays a key role in the Algoma Diabetes Education and Care Program (ADEC) and Cardiac Rehabilitation Program (CR). As part of these innovative programs, the Social Worker provides essential psychosocial support, counseling, education, and resource navigation to patients and families affected by chronic illness, mental health concerns, and the challenges of adjusting to major health diagnoses.

This position requires a compassionate and patient-centered approach to healthcare, with a focus on supporting patients through the emotional and behavioral aspects of living with chronic conditions such as diabetes and heart disease. The Social Worker will work collaboratively with an interdisciplinary team to ensure holistic care, optimize patient health, and promote overall wellness in both individual and group settings.

Essential Duties & Responsibilities

Patient Support & Counselling

  • Provide individual and group counseling for patients and families participating in the ADEC and CR programs.
  • Offer psychosocial support to help patients adjust to new diagnoses and manage lifestyle changes associated with chronic illness.
  • Assist patients and families in coping with mental health concerns, including anxiety, depression, and stress related to chronic disease.
  • Provide education on coping strategies, behavior modification, and self-management techniques for chronic conditions.

Collaboration & Interdisciplinary Care

  • Collaborate closely with an interdisciplinary team to deliver comprehensive, patient-centered care.
  • Work with patients, families, caregivers, and community resources to facilitate access to essential supports and services that enhance overall health.
  • Participate in care planning meetings and provide valuable insights to optimize patient care and treatment plans.

Community Resource Navigation

  • Assist patients in accessing community-based resources to support their health and wellness, including counseling services, financial support, and local health programs.
  • Provide advocacy and navigate complex social service systems to meet patients’ diverse needs.

Clinical Assessments & Education

  • Conduct psychosocial assessments to evaluate the emotional, social, and psychological needs of patients.
  • Deliver education sessions to patients, families, and groups, focusing on mental health, chronic disease management, and coping skills.
  • Lead group sessions focused on chronic disease self-management, motivational interviewing, and other evidence-based therapeutic approaches.

Documentation & Reporting

  • Maintain accurate and comprehensive patient records, including clinical assessments, treatment plans, and progress notes, in compliance with organizational standards.
  • Contribute to the overall evaluation and reporting of the ADEC and CR programs.

Required Qualifications

Education & Licensure/Certification

  • Bachelor’s Degree in Social Work (BSW)
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers (OCSWSSW)

Knowledge, Skills, & Abilities  

  • Strong psychosocial assessment skills, including the ability to evaluate patients’ emotional and psychological needs.
  • Knowledge and application of social work values, ethics, theory, and clinical practices in an outpatient healthcare setting.
  • Proficiency in evidence-based therapeutic modalities, such as Cognitive Behavioral Therapy (CBT), solution-focused therapy, and motivational interviewing.
  • Competence in the classification and treatment of psychiatric illnesses such as anxiety, affective disorders, OCD, personality disorders, and depression.
  • Experience in planning and delivering social work interventions, both individually and in group settings, with patients, families, caregivers, interdisciplinary teams, and community resources.
  • Knowledge of and ability to access community resources to support patients and families.
  • Experience in delivering individual and group education sessions focused on chronic disease management, mental health, and wellness.
  • Strong interpersonal and communication skills, both verbal and written, with the ability to interact effectively with patients, families, and team members.
  • Ability to work independently and as part of an inter-professional team to ensure holistic, patient-centered care.
  • Effective prioritization, organization, and conflict resolution skills.
  • Strong decision-making and problem-solving abilities, especially when working under pressure or with complex cases.
  • Commitment to ongoing professional development, keeping up-to-date with best practices in social work and healthcare.
  • Computer literacy, including proficiency in word processing, database management, and experience with Electronic Medical Records (EMR) systems (experience with EPIC is an asset).
  • Dedication to promoting evidence-based, patient-centered care that focuses on wellness and optimal health outcomes.
  • Understanding of consent and capacity legislation, ensuring ethical and legal standards are met in patient care.
  • Adherence to safety policies and procedures to maintain a safe and inclusive environment for all patients and staff.
  • Empathy in interactions with both patients and staff, demonstrating compassion and respect in all encounters.
  • Training in CBT, crisis intervention, motivational interviewing, and chronic disease self-management is an asset.
  • Knowledge of chronic diseases including diabetes and cardiac events, with the ability to integrate this knowledge into patient care plans.
  • Training in pediatric and family therapy is an asset, with the ability to support families in navigating health challenges together.
